The Revenue have placed the updated form, which is required as an affidavit or affirmation for Revenue where an application is made for a second or subsequent grant of representation, on their website.

In general the form is required where the original personal representative has died or where further assets have been discovered since the original grant was sworn.

The updated form can be downloaded from http://www.revenue.ie/forms/a3c.pdf and can then be completed on screen.
